1.0 PURPOSE: To establish a process for evaluating patient access needs and preferences. 2.0 POLICY: Each facility in the Ambulatory Care Network (ACN) will evaluate patient access by utilizing collected data by means of a department-wide standardized and validated survey tool that will measure if existing access methods are adequate for its population. 3.0 PROCEDURE: Each facility will review access to care survey questions on a quarterly basis to determine if current methods of access are sufficient to meet the needs of the patient population being served. 4.0 QUALITY IMPROVEMENT:
4.1 The department-wide validated survey tool quarterly report will be reviewed by the facilities’ leadership and quality improvement teams. 4.2 Data will be reviewed in areas where improvement to access to care is needed and implement systematic changes are required. 4.3 The data will also be reviewed at Quality Board on a quarterly basis.
