Steps for Obtaining a Leave of Absence
  1. Review the Leave of Absence (LOA) policy.
  2. Obtain an LOA Form from the Registrar’s Office.
  3. Schedule an appointment with the following persons to obtain the signatures required by the LOA Form:
    1. Academic Advisor
    2. Director of Business Services, or designee
    3. Bursar, or designee
    4. Program Director, where applicable.
  4. Make an appointment to meet with the Dean of Student Affairs.
    • When meeting with the Dean, the student will submit the completed LOA Form and discuss the next steps to be taken (including what supporting documentation will need to be submitted).
